• Reside in Hudson and/or Essex County NJ
• Identify problems and provide care management interventions
• Coordinate care between members, caregivers, and providers
• Provide ongoing follow-up and monitoring of member status and progress towards care plan goals
• Offer resource support for local services, such as employment and housing
• Facilitate care management and provide timely access to needed care
• Collect and maintain member information and care management activities
• Provide education to members and their families on disease processes and care options
• Provide feedback to leadership on opportunities to improve care deliveryMust reside in Hudson and/or Essex County NJEducation/Experience: Bachelor's degree in Nursing and 2 - 4 years of related experience. RN - Registered Nurse - State Licensure and/or Compact State Licensure required.Pay Range: $54,000.00 - $97,100.00 per yearCentene offers a comprehensive benefits package, including competitive pay, health insurance, and a flexible approach to work.
